The Elmhurst Clinic is already on the forefront of the next steps in primary care. We’re working to create a medical home for you, one that improves quality, enhances access and reduces costs to all of those that are involved. 

This concept of a Patient-Centered Medical Home focuses on managing chronic conditions better as well as streamlining the care you receive.  While we already do many of these things, we will be working to do more and take things to the next level by serving you the best we can. 

We see this idea of having a relationship with a physician who takes a whole-person approach to your care - at all levels of health - as the next step in medicine. So we’re getting started now.
