The Elmhurst Clinic is on the forefront of the next steps in primary care. We’re creating a medical home for you, one that improves quality, enhances access and reduces costs to all of those that are involved. 

This concept of a Patient-Centered Medical Home focuses on managing chronic conditions better as well as streamlining the care you receive.  While we already do many of these things, we will be working to do more and take things to the next level by serving you the best we can. 

We see this idea of having a relationship with a physician who takes a whole-person approach to your care - at all levels of health - as the next step in medicine. And our efforts have been recognized.

Not only were we selected to participate in a pilot program with Blue Cross-Blue Shield of Illinois, a number of our physicians also became the first in the state to receive recognition from the National Committee for Quality Assurance for functioning as a medical home. Click here to learn more.
