How do I make an appointment?
Appointments can be made by calling the appropriate medical department telephone number listed below. If you need to make an appointment with a physician other than your own, ask the appointment secretary to schedule you with someone who is appropriate for your medical needs. If you need immediate care, you will be promptly scheduled. We will make every effort to provide all patients with timely and thorough medical treatment.

How do I reach the Elmhurst Clinic?
To reach us, call the appropriate medical department telephone number listed above. They will make it easier for you to reach your physician’s office directly. A receptionist will ask for your name, telephone number and the nature of your call. If necessary, you may be asked to provide your birth date, any known allergies and current insurance information. If your call is not urgent, your physician will return it as soon as possible or after office hours.
What if I have to cancel my appointment?
If you are unable to keep an appointment, please notify the appropriate appointment desk at least 24 hours in advance. Give the name of your physician and the date and time of your appointment. You may reschedule your appointment at that time.
How do I get my prescriptions refilled?
Please try to call for prescription refills before your prescription runs out. Have the name and number of the prescription, the pharmacy’s phone number and dosage of medication available when you call. Pharmacies operated by Elmhurst Memorial Hospital are located at the Elmhurst Memorial Hospital campus and the Elmhurst Memorial Center for Health. They are available for your use. Pharmacy charges, however, will not appear on your Clinic billing statement.
How do I notify my doctor in an emergency?
In an emergency, call your Elmhurst Clinic physicians at the appropriate medical department telephone number or at 630-834-1120. All phone numbers transfer to an answering service after hours.
How do I get a copy of my medical records?
A personal medical record of all medical care received at the Elmhurst Clinic is maintained for each patient. It provides a history of treatment, medication and diagnostic information that will enable your physician to make comprehensive ongoing evaluations of your health. Your medical record also ensures that the treatments you receive from any Clinic physician are compatible. Your physician may ask you for information about previous medical care to supplement your Elmhurst Clinic medical records. Your medical record is confidential. Information will not be disclosed without your written consent. Although the medical record is the property of the Elmhurst Clinic, you may request a copy. Contact our Medical Records department at 630-758-9719. There is a charge for the cost of duplication. You will be required to sign a release form and can expect the copies to be ready within five working days. Emergency situations are expedited.
